Immunization Record

Immunization Requirements 

New Jersey law (N.J.S.A 18A:61D-1), as a condition of admission or continued enrollment, require every student who is 30 years of age or less and is enrolled full-time or part-time in a program or course of study leading to an academic degree, to submit to the college a valid immunization record which documents the administration of all required immunizations against vaccine-preventable disease, or evidence of immunity from these diseases, in accordance with regulations promulgated by the Department of Health. Provisional admittance may be granted for 1 term (no more than 4 months).

Acceptable Proof of Immunization

  • Government record
  • Records from licensed health care professionals
  • Official school immunization records
  • Foreign records, if accompanied by a certified English translation.  Any adult translation is acceptable but must be accompanied by the SCCC Translation of Foreign Immunization Records form. 

Measles, Mumps and Rubella (MMR) Vaccine

Required for all students 30 years of age or younger. 

Hepatitis B Vaccine

All full-time students enrolling at a public or private institution of higher education in this state must be vaccinated for Hepatitis B (based on recommended schedule). Students needing to complete the series will have nine months to provide proof of Hepatitis B immunizations or proof of immunity. Students 31 and older may be subject to the requirement if indicated.

Meningococcal Vaccine

All full-time and part-time students enrolling at a community college of higher education in this state shall be vaccinated for Meningococcal ACWY (based on recommended schedule). Students 31 or older may be subject to the requirement if indicated.

Exemptions

  • Religious Exemption (N.J.S.A. 26:1A-9.1 and N.J.A.C. 8:57-4.4): Students whose religious principles prohibit injections may request a religious exemption. An essay of explanation is required and must be signed and dated. 
  • Medical Exemption (N.J.A.C. 8:57-4.3): Students may request a medical exemption if the vaccine is medically contraindicated; a statement by a licensed physician is required. Please refer to the form created by the State of New Jersey for medical professionals to complete for a medical exemption.  Beginning the 2026-2027 school year, all medical exemptions must be submitted on the official IMM-53 form (Request for Medical Exemption from Mandatory Immunization Form).  All medical exemptions must be reviewed annually or when the contraindication/precaution dated ends, whichever comes first. All renewals must be submitted on an IMM-53 form.  Please refer to the form created by the State of New Jersey for medical professionals to complete for a medical exemption:
  • Philosophical and moral objections are not accepted by the State of New Jersey.

In the event of an outbreak

N.J.A.C. 8:57-4.19 Emergency powers of the Commissioner of Health 鈥�

(a) In the event that the Commissioner, Department of Health or his or her designee determines either that an outbreak or threatened outbreak of disease or other public health immunization emergency exists, the Commissioner or his or her designee may issue either additional immunization requirements to control the outbreak or threat of an outbreak or modify immunization requirements to meet the emergency.

(b) All children failing to meet these additional requirements shall be excluded from a school, preschool, or child care center until the outbreak or threatened outbreak is over.

(c) These requirements or amendments to the requirements shall remain in effect until such time as the Commissioner, Department of Health or his or her designee determines that an outbreak or a threatened outbreak no longer exists or the emergency is declared over, or for three months after the declaration of the emergency, whichever one comes first. The Commissioner, Department of Health, or his or her designee may re-declare a state of emergency if the emergency has not ended.

(d) N.J.A.C. 8:57-4.4 Religious exemptions - Those children with religious exemptions from receiving immunizing agents may be excluded from the school, preschool, or child care center during a vaccine-preventable disease outbreak or threatened outbreak as determined by the Commissioner, Department of Health or his or her designee.

(e) N.J.A.C. 8:57-4.3 Medical exemptions - Those children with medical exemptions to receiving specific immunizations may be excluded from the school, preschool, or child care facility during a vaccine-preventable disease outbreak or threatened outbreak as determined by the Commissioner, Health or his or her designee.

(f) N.J.A.C. 8:47-4.5 Provisional admission - Those children in provisional status may be temporarily excluded from the school, preschool, or child care center during a vaccine-preventable disease outbreak or threatened outbreak as determined by the Commissioner, Department of Health or his or her designee.

 

Full-time, matriculated students who have not fulfilled all vaccination requirements may be barred from full-time registration after their first semester.

Pursuant to the Administrative Procedure Act, P.L.1968, c.410 (C.52:14B-1 et seq.), if a student is unable to obtain immunization records and does not have a primary care physician, 猫咪社区 will refer students in need of the Measles, Mumps, Rubella, Hepatitis B and Meningococcal immunizations to the Atlantic Health system to explore the opportunity to receive the required vaccinations through Atlantic Health鈥檚 services in Sussex County. Atlantic Health will work with the students for possible health insurance coverage or other means of payment. As a part of this agreement, 猫咪社区 is not responsible for the costs incurred by the student.
